  - USYS: Teams should obtain "Application for Travel" forms from their own state offices.
  - US Club Soccer:  Not required for teams registering with US  Club Soccer credentials.

Fort Dix: All Boys Teams, U11 Girls Cup flight, U13 Girls Elite (Sunday), U14 Girls Elite (Sunday)
Bordentown: U9 Girls, U10 Girls, U11 Girls, U12 Girls

 Print a copy of your completed application and confirmation, and mail both with your check
