REQUIRED REGISTRATION MATERIALS:

US Club Soccer Teams

    1.  US Club Roster – original and 2 copies
     2.  Valid US Club Player Passes
     3. 
US Club R002 Medical Treatment forms
     4.  Guest players must have valid US Club Player Pass
 

US Youth Soccer Teams

    1.  Original State Approved Roster and 2 copies
    
2.  Validated US Youth Soccer Player Passes – includes Guest Players
     3.  Current, signed Medical Release form for every player –
             includes Guest Players (maximum of 5).

Out-of-state teams -- Permission to Travel Form*

Out-of-state teams should obtain "Application for Travel" forms from their own state offices.
     5.  Guest players must have valid USYS player pass,
             medical release form, and permission to travel from your state
    
6.  Guest Player forms as required by your state association
            

International Teams

    1.  Original Provincial Approved Roster and 2 copies
   
2.  Validated player passes
    3. 
Current, signed Medical Release form for every player –
             includes Guest Players (maximum of 5)– forms do not have to be notarized

    4. 
Guest Player forms as required by your Provincial association
    5.  Permission to Travel Form if required by your Province
    6.  Canadian teams must provide proof of legal entry into the U.S.
